24-Hour Emergency Assistance

24-Hour Supports is a service that allows for people to connect to a live person 24 hours a day, 7 days a week to decrease anxiety and mitigate challenges that may occur outside of normal work hours.

Some ways that 24-Hour Supports can help include:

  • Decreasing anxiety, large and small, through talking with someone
  • Being able to communicate frustrations or challenges with your team
  • Getting help with situations that happen outside of normal work hours
  • Talking to someone to help decide if a situation is dangerous or needs more support

Philosophy of the Program: At Oak Tree Support Services our mission is to eradicate homelessness by meeting people where they are and helping them get to where they want to be through county and federal programs including housing, employment and independence supports. Through person-centered tools and planning, we support people to achieve the change they want while embodying unwavering integrity with a joyful presence.

 

Program Description: At Oak Tree our 24-hour Emergency Services provides a phoneline that is available 24-hours a day to help people problem solve issues that arise including mental, physical or social emergencies.

 

Level of Care: Phone and in person consultation, as needed, based on the circumstances.

 

Program Goals: Oak Tree Support Services strives to help the person served connect with community and personal support system, while helping each person served increase independence. 24 hour emergency assistance helps individuals achieve this goal by providing them consultation as emergency situations arise to help mitigate those situations to the best of our ability.

 

Treatment or Service Modalities: Phone and in person consultation as needed based on the circumstances.

 

Population Served: At Oak Tree Support Services we provide 24-hour Support Services to individuals on the Minnesota Medicare Waiver Program.

 

Credentials of Those Providing Care: Our staff receive initial and ongoing training as required in Minnesota Statute 245D, likewise the Program Director must qualify as a Designated Coordinator/Designated Manager per 245D.

 

Settings: Oak Tree offers a range of settings to assist the persons served, from within that person’s home, to out in the community, or from a remote location.

 

Hours of Service: 24/7/365

 

Payors, Fees and Funding Source: We are funded through the Minnesota Medicare waiver system.

 

Referral Sources: Case Manager, personal referrals, internal referrals, and other community partners.
