Housing Stabilization Services Overview

Housing Stabilization Services is a service through Medical Assistance to help those 18 and older at risk of, or experiencing, homelessness with a mental or physical diagnosis find and keep housing. The purpose of Housing Stabilization is to support people find housing, walk alongside them through the moving process, increase long-term stability within the community, and decrease risk of future homelessness. There are two parts of HSS, transition and sustain. A person can choose to participate in part or both parts.

Housing Stabilization Services- Transition

Transition services includes guiding people through evaluating housing wants and needs, finding available options through housing search using person-centered preferences, navigating the application process, assist with alleviating barriers, and walking alongside them through the moving process.

Housing Stabilization Services- Sustain

Sustain services support people after moving into independent housing or while remaining in already secured housing to provide supports that encourage continued stability. This can look differently for every person's situation but can include assisting building connections within the community, helping a person reach self-determined goals, and advocate on behalf of the person when challenges arise.

HSS Referral Process

If a person has a CSSP, that is the only documentation that is needed. This document shows proof of disability, proof of assessment, and is also considered a person-centered plan.

If a person does not have a waiver but does meet the qualifications for HSS they will need one document that satisfies each of the following categories:

  • Referral form from Oak Tree Support Services
  • Proof of Medical Assistance: PMI or copy of MA card
  • Assessment:
    • MNChoice assessment or
    • PSN or
    • Coordinated Entry receipt number
  • Proof of disability:
    • PSN or
    • medical opinion form or
    • proof of SSI/SSDI
  • Person Centered Plan:
    • CCP (seniors) or
    • Housing-Focused Person-Centered Plan (completed through HSS consultation)

Oak Tree Support Services

TMS (Transitional Moving Services)

TMS is a great service provided by waivers that allows for payment and facilitation of movers, moving truck, moving supplies, and a small pool of money for furniture and household goods. This service can be accessed once every three rolling years.

HSS Additional Details:

Philosophy of the Program: At Oak Tree Support Services our mission is to eradicate homelessness by meeting people where they are and helping them get to where they want to be through county and federal programs including housing, employment and independence supports.

Through person-centered tools and planning, we support people to achieve the change they want while embodying unwavering integrity with a joyful presence.

 

Program Description: At Oak Tree our Housing Stabilization Services is broken down into 2 categories.  Transition services which provides assistance finding and securing housing.  They make arrangements for moving, help with packing and help find funds for the costs of moving into a new living arrangement.  Support services are ongoing to help maintain the stability of the persons new home.  Services can range from problem-solving a disagreement with a neighbor to helping find and apply for assistance.

 

Level of Care: In person and virtual contact with the person served based on their preference, along with important people in their lives, along with indirect work on their behalf.

 

Program Goals: Oak Tree Support Services strives to help the person served connect with community and personal support system, while helping each person served increase independence. Housing Stabilization Services serves to achieve this goal by helping people obtain and maintain stable, independent housing.

 

Treatment or Service Modalities: In person and virtual contact with the person served based on their preference, along with important people in their lives, along with indirect work on their behalf.

 

Population Served: At Oak Tree Support Services we provide Housing Stabilization Services to those who are on a Minnesota Medicare waiver as well as anyone who is 18 years of age or older on MA (Medical Assistance) that qualify for Housing Stabilization Services

 

Credentials of Those Providing Care: Our staff are required to meet initial and ongoing training as mandated for Minnesota HCBS providers and Oak Tree mandated training.

 

Settings: Oak Tree offers a range of settings to assist the persons served, including the person’s home and in the community.

 

Hours of Service: Our hours of operations are Monday through Friday 9 AM to 8 PM, but may vary from that depending on the client’s needs. Emergency information is provided to all clients.

 

Payors, Fees and Funding Source: We are funded through  the Minnesota Medicare waiver program as well as anyone who is 18 years of age or older that are on MA (Medical Assistance)

 

Referral Sources: Case managers, personal referrals, internal referrals and other community partners.

Program Description: At Oak Tree our Transitional (Moving) Services is a service that is available to provide individuals utilizing Minnesota Medicare waivered service assistance moving to independent housing.  Additionally, the program can be authorized to provide limited furniture and household items as well as pay for deposits on new homes.

 

Program Goals: Oak Tree Support Services strives to help the person served connect with community and personal support system, while helping each person served increase independence. Transitional (Moving) Services serves this goal by helping people move into independent housing.

 

Treatment or Service Modalities: TMS staff work with individuals in-person, virtually, as well as do indirect work for them.

 

Population Served: At Oak Tree Support Services we provide Transitional (Moving) Services to individuals that are on a Minnesota Medicare waivered program.

 

Credentials of Those Providing Care: Our staff complete orientation and annual training as required for Minnesota HCBS providers.

 

Settings: Oak Tree offers a range of settings to assist the persons served, from within that person’s home that they are moving out of as well as the house that they are moving into, as well as virtually and indirectly.

 

Hours of Service: Our hours of operations are Monday through Friday 9 AM to 8 PM, but may be altered due to client’s needs.  Emergency information is provided to all clients.

 

Payors, Fees and Funding Source:  Our Transitional (Moving) Services is funded through the Minnesota Medicare waiver program.
