IHS: Individualized Home Services

IHS with training (Individualized Home Services with training)

A person receiving this service has access to an array of different types of assistance. Staff are available up to 16 hours a day in 4 categories: community participation, health and wellness, household management, and adaptive skills. Services can be received in the home and out in the community. Staff can provide help coordinating appointments, cues for self-cares, coordination of community living activities, independence skill building, budgeting, and crisis prevention skills as well as assistance with many other daily living tasks.

IHS Remote (Remote Individualized Home Services)

Through IHS remote, staff are available to communicate with the person via phone or text to problem solve, resolve anxiety, or help reduce social isolation. Communication can be initiated by either the person or the staff. Remote IHS can be authorized for up to 2 hours per day and can be used flexibly. This service can be used in tandem with IHS or independently.

Philosophy of the Program: At Oak Tree Support Services our mission is to eradicate homelessness by meeting people where they are and helping them get to where they want to be through county and federal programs including housing, employment and independence supports. Through person-centered tools and planning, we support people to achieve the change they want while embodying unwavering integrity with a joyful presence.

 

Program Description: At Oak Tree our In-home services are a service funded through the Minnesota Medicare waiver program. The primary service is Individualized Home Services (IHS).  IHS is a broad-scoped service platform that includes community participation, health, safety and wellness, household management and adaptive skills.

 

Level of Care: IHS is a broad-scoped person-centered service that includes community participation, health, safety and wellness, household management and adaptive skills.

 

Program Goals: Oak Tree Support Services strives to help the person served connect with community and personal support system, while helping each person served increase independence. Independence Services serves to achieve this goal by assuring that individuals supported can continue to live independently.

 

Treatment or Service Modalities: In person and virtual support.

 

Population Served: At Oak Tree Support Services we provide IHS services to individuals on the Minnesota Medicare waiver program.

 

Credentials of Those Providing Care: All training required by Minnesota Statute 245D and Oak Tree Support Services

 

Settings: Oak Tree delivers services where the person would prefer them, this can be in there home or in the community.

 

Hours of Service: Staff are available to work with individuals as determined by the persons wants and needs.

 

Payors, Fees and Funding Source: We are funded through the Minnesota Medicare waiver program.

 

Referral Sources: Case managers, internal and self referrals, along with other community partners.
