LifeSharing-Matching Overview
LifeSharing-Matching Services is available for Case Managers and Persons Served alike. LifeSharing-Matching, at its basis, is a service provided to individuals looking for placement into Family Residential Services or prefer to have a live-in caregiver. Our staff use person-centered practices to work with the individual and find a home that fits their needs and wants.
Benefits of LifeSharing-Matching
The Placement Coordinator assists with research and locates potential placement sites for the individual to visit, helps the individual get to know the area prior to moving in, assists with moving, is present on the day of move-in, and continues to provide communication and visits until the Person Served is settled and comfortable. Having that familiar face working with you throughout makes it easier for all parties!
Benefits for the Person Served:
- Provides the opportunity for person-centered research and placement.
- Gives the Person Served an opportunity to visit multiple homes and get to know potential providers.
- Provide Person Served with assistance throughout the location and moving process. (Everything from finding a new pharmacy and transferring prescriptions to locating the closest shopping sites for an individual. Individuals are assisted with packing and moving as able.
- Persons Served are the driving force for this service. Individuals are helped to create all aspects of their new home, from location to proximity of family, public transportation, shopping, or work, whatever is important to/for them.
- An individual can use the matching process to find a roommate for their own home.
- An individual can use the matching process to find a live-in caregiver who provides them with ongoing support through caregiver living expenses.
- An individual can use the matching process to find a family foster care provider.
- The Person Served is given the opportunity to get to know the new provider prior to moving in. Stress is eliminated, which allows for a smoother transition.
Benefits for the Case Manager:
- Takes the task of researching, finding, and placement of a client in a new home. (Roommate, FRS, Live-in caregiver)
- Provides transportation to visit multiple sites along with a Placement Coordinator to coordinate.
- LifeSharing-Matching is a HUGE time saver for Case Managers who are already overworked!
LifeSharing-Matching Referral Process
LifeSharing-Matching is easy to use!
If you're on a waiver and looking to:

- Find a roommate, or
- Looking for a Live-In Caregiver, or
- Move into a Family Residential Setting.
We'll simply need:

- A referral form to Oak Tree Support Services, and an
- Active Service Agreement.
